Post Hospitalization and Post Surgical Care
Post-acute care is a crucial component of our service plan for patients recovering from hospitalization. This term refers to a variety of medical services provided after a stay in an acute care facility. Often, patients are discharged from acute care before they are fully healed, as their ongoing needs can be better addressed in another type of facility. However, securing post-acute care can be nearly impossible for those experiencing homelessness, leaving them vulnerable following surgery or other medical procedures.
Post-acute care encompasses a range of services and can be provided through inpatient rehabilitation facilities, long-term hospitals, skilled nursing facilities, or recuperative care centers like ours. We offer comprehensive post-acute care, including pain management, wound care, physical rehabilitation, speech rehabilitation, post-surgical care, and assistance with daily living tasks. Our goal is to ensure that our patients receive the necessary support and care to achieve a full and healthy recovery.
